1. This promotion is reserved for customers apply for Allianz Travel Annual Plan through allianz.co.th website from 1 January – 31 December 2024 only and the policy must not be cancelled before travel.
2. Only the insured person who completes a form https://forms.office.com/e/Hu2Q7HZ8XE within 15 January 2025 can redeem the Starbucks card.
3. When the insured apply for travel insurance according to clause 1, the insured will receive the following rewards:
4.         - Premium 4,000 – 6,000 THB per person and Starbucks card 400 THB
5.         - Premium 6,001 – 7,000 THB per person and Starbucks card 500 THB
6.         - Premium 7,001 – 9,000 THB per person get Starbucks card 600 THB
7.         - Premium 9,001 – 18,000 THB per person and Starbucks card 800 THB
8.         - Premium 18,001 THB or more per person and get Starbucks card 1,000 THB
   
   
9. The Company’s decision is final and conclusive. If the participant is found to be fraudulent in any form, the Company may disqualify the prize without prior notice.
10. The promotion is reserved for lawful residents of the Kingdom of Thailand only.
11. In case of cancellation of the policy, the company reserves the right to recall Starbucks cards as specified in the promotion.
12. The prize is non-exchangeable, non-transferable and not redeemable for cash. Terms and conditions of Starbucks Card are as specified by Starbucks Thailand.
13. Starbucks cards will be mailed within 15 working days after the date the insured starts traveling according to the policy, send to the name (the recipient’s name must be the name of the insured specified in the policy) and the address specified in the registration form only. The company reserves the right to send Starbucks card in case of incorrect name or contact address.
14. Please ensure you carefully read and understand the details of the insurance policy prior to making a decision to purchase this insurance.
15. The company reserves the right to amend, change, or cancel the terms and conditions without prior notice. In case of any dispute, the decision of company shall be final
16. Where these terms and conditions are in Thai and English language, if there are any inconsistencies or discrepancies. Thai version shall be prevailed for interpretation.
