Personal Accident Insurance & Health Insurance (with TPA Logo card and MED SURE card)
Documents Required
Procedures
- In case of outpatient (OPD) treatment that is not an exception case, the hospital is to verify coverage
- In case of inpatient (IPD) treatment, hospital personnel coordinates with the company’s staff or agent to service the Insured as per coverage stated in the insurance policy.
- Should there be any inconvenience getting service from hospital, please contact the number on the back of your insurance card
Condition
- The company reserves the right for the Insured with TPA Logo card and MED SURE card and getting treatment at Allianz Ayudhya Care Network hospitals only.
- In case of no card or getting treatment at a hospital outside the Allianz Ayudhya Care Network, the Insured is to make advance payment and submit documents to the company for reimbursement afterwards.
- Indemnity payment can be transferred to savings account of any bank except Government Savings Bank, Islamic Bank of Thailand, Bank for Agriculture and Agricultural Cooperatives, and Government Housing Bank. A cheque payment will be made if the Insured does not have a passbook.
Personal Accident Insurance Claim (without Insurance Card)

- In case of reimbursement for broken bone treatment, radiographic image and X-Ray/CT Scan/MRI results report are required
- Copy of the first page of the Insured’s passbook (for indemnity payment transfer)
- Other documents as requested by the company
