Personal Accident Claim 

Personal Accident Insurance & Health Insurance (with TPA Logo card and MED SURE card)

Show health insurance card together with identification card or any other government-issued card to receive treatment at a hospital in Allianz Ayudhya Care Network.
  1. In case of outpatient (OPD) treatment that is not an exception case, the hospital is to verify coverage
  2. In case of inpatient (IPD) treatment, hospital personnel coordinates with the company’s staff or agent to service the Insured as per coverage stated in the insurance policy.
  3. Should there be any inconvenience getting service from hospital, please contact the number on the back of your insurance card
  1. The company reserves the right for the Insured with TPA Logo card and MED SURE card and getting treatment at Allianz Ayudhya Care Network hospitals only.
  2. In case of no card or getting treatment at a hospital outside the Allianz Ayudhya Care Network, the Insured is to make advance payment and submit documents to the company for reimbursement afterwards.
  3. Indemnity payment can be transferred to savings account of any bank except Government Savings Bank, Islamic Bank of Thailand, Bank for Agriculture and Agricultural Cooperatives, and Government Housing Bank. A cheque payment will be made if the Insured does not have a passbook.

Personal Accident Insurance Claim (without Insurance Card)

  1. Original copy of the receipt and medical certificate
  2. In case of reimbursement for broken bone treatment, radiographic image and X-Ray/CT Scan/MRI results report are required
  3. Copy of the first page of the Insured’s passbook (for indemnity payment transfer)
  4. Other documents as requested by the company

Death Claim

  1. Copy of death certificate or autopsy report
  2. Certified copy of the Insured’s identification card and house registration
  3. Certified copy of the beneficiary’s identification card and house registration
  4. Copy of marriage certificate (if applicable) or divorce certificate (if any)
  5. Copy of police daily log about the case, certified by case officer (if any)
  6. Copy of alcohol test report (if any)
  7. Record of family tree
  8. Copy of the first page of the Insured’s passbook (for indemnity payment transfer)
  9. Other documents as requested by the company

  3. Certificate of disability and identification card for disabled person (in case of disability)

  5. Other documents as requested by the company

  2. Copy of tissue diagnosis lab tests/reports
  3. Copy of medical treatment records from all hospitals at which the Insured has been treated

  5. Copy of the first page of the Insured’s passbook (for indemnity payment transfer)
