  4. Photos of damaged goods
  5. Repair quotation, requests for claims to and responses from related parties e.g. carrier/vessel, airline, transporter and forwarder (upon request from the company)
  6. Copy of the first page of the Insured’s passbook (for indemnity payment transfer)
  7. Other documents as requested by the company

Indemnity payment can be transferred to savings account of any bank except Government Savings Bank, Islamic Bank of Thailand, Bank for Agriculture and Agricultural Cooperatives, and Government Housing Bank. A cheque payment will be made if the Insured does not have a passbook.

  4. Description of goods and prices

  6. Copy of driver’s driving license

  8. Receipt, repair quotation
  9. Copy of police daily log certified by case officer (if any)
  10. Copy of the first page of the Insured’s passbook (for indemnity payment transfer)
  11. Other documents as requested by the company

  3. Copy of ship’s license
  4. Copies of ship master and helmsman certificates
  5. Copies of ownership transfer/mortgage documents
  6. Copy of police daily log (if nay)
